How to Register
To register with our practice please complete the 4 following steps:
- Check you are living within our catchment area. Link as mentioned above. If you are within the area then only proceed to number 2.
- Please complete GMS1 form. This form is mandatory to fill.
- Next, please complete one of these forms : For 16 years and over : Adult Registration Form and for 0-16 years : Child Registration Form.
- Make sure you have filled both forms mentioned in point 2 and 3.
Non-urgent advice:
We will not accept the forms if you are living out of catchment area and those forms will be shredded.
Without GMS1 form, if only Adult / Child registration form is filled, practice will not be able to register you and these forms will be shredded.
After you have completed and submitted both forms you must send 2 forms of identification (ID) to the practice in 10 working days. Contact the reception on 020 85781880 for more information on how to submit ID.
If you do not contact the practice in 3 working days, we will presume you have withdrawn your registration with us.
Non-urgent advice:
Acceptable Identification: tenancy agreement, mortgage agreement, bank statement, driving license, passport, birth certificate etc.
When you come in with your ID then we will issue with online login details.
Registration Medical
Once registered, make an appointment with our nursing team for a new patient health check.
We invite all our new patients to see one of our practice nurses or our health care assistant. This appointment enables us to collect medical information before your medical records arrive. It is also the start of screening for other medical problems.
